Mechanism of Action
Melanotan-I (Afamelanotide) is a synthetic linear alpha-MSH analogue that selectively activates MC1R on melanocytes, stimulating eumelanin production through cAMP-PKA-CREB-MITF cascade. FDA-approved (2019) as Scenesse for erythropoietic protoporphyria (EPP).
Key Research Findings
- FDA-approved for EPP: Significantly increased pain-free sun exposure time
- Long-term observational data (8 years, 115 patients) showed sustained quality of life improvement
- Combination with NB-UVB produced superior repigmentation in vitiligo patients
